New Patient Registration

 

Welcome to kilburn Park Medical Centre, if you are a new patient who is looking to register with us, you can do so by completing our online registration form below. 

We need to check that you live within our surgery's boundary area. To check this simply type your postcode in the checker below. If you are within our boundary you will be given a link to our registration form.

Our patient registration policy does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.

How To Register?

To register as a new patient at the practice. Please follow the registration process below.

Complete a New Patient Registration Form

If you live within the boundary area, please complete our new patient registration.

Where it is clinically appropriate and practical to register, we now accept new registrations from patients who work in the local area but reside outside of our registration area. Patients registered this way would not be entitled to home visits from the practice, however, they will be able to contact NHS 111 in order to be seen by a practice closer to where they live.

Register at our GP Practice

You will be asked to share two forms of ID to help us correctly identify you. 

Acceptable identification includes:

  • Passport
  • Birth Certificate
  • HC2 Certificate
  • Rough sleepers’ identity badge
  • Hostel Registration/mail forwarding letter.

We also need proof of address which can be a utility bill like a water bill or energy bill dated within the last 3-months.

Until we have seen your identification you will not be registered at the practice.

Registering for Online Services

You can use the Internet to book and cancel an appointment with a GP, request repeat prescriptions and view aspects of your medical record online.

If you wish to register for this service, please use the application for online access form.

Your Named GP

All Patients registered with the surgery will have been allocated a named accountable GP who will be responsible for your overall care. However, you can book appointments with any of the GPs at the surgery. If you have a preference to which GP you are allocated, the practice will make reasonable efforts to accommodate your requests. If you wish to know which GP has been allocated to you, please ask reception at your next visit.
